Travel Permits

 


Travel Outside Alberta, within Canada

For travel outside of Alberta (within Canada) please submit a Travel Permit request and team roster to the CMSA office a minimum of three weeks prior to departure.

(release letters are required for any players not registered with your team)


Travel Fee

  • There is a $15.00 fee payable to CMSA for all travel permit requests.

  • Revisions: a $15.00 administrative fee will be in place each time a revised roster is submitted. No changes are permitted to rosters one week before departure ($50 fee may apply).

  • Late fees: Travel permits received less than two weeks prior to travel will be subject to the following late fees plus the usual travel fee:

    • ASA’s $50 administrative fee

    • CMSA’s $50 administrative fee

    • Travel permits received less then 72 hours prior to travel will be subject to a $150 administrative fee plus a $100 CMSA administrative fee.


Travel within the United States

For travel to the United States, please submit a Travel Permit request, team roster, mandatory travel insurance registration form to the CMSA office a minimum of four weeks prior to departure.

International Travel

  • Revisions: a $20.00 administrative fee will be in place each time a revised roster is submitted. No changes are permitted to rosters one week before departure ($50 fee may apply).

  • Late fees: Travel permits received less than two weeks prior to travel will be subject to the following late fees plus the usual travel fee:

    • ASA’s $50 administrative fee

    • CMSA’s $50 administrative fee

    • Please note that we cannot guarantee that any travel permits for travel outside of Canada and the USA will receive sanctioning by CONCACAF (via Canada Soccer) should they be submitted later than six weeks prior to travel. 


Submissions

Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to slarison@calgaryminorsoccer.com. Payment can be made in person by cash, cheque, Visa, MasterCard, debit card or over the phone during regular office hours by Visa or MasterCard.
