Handwritten rosters and travel permits will not be accepted.
Forms
Travel Permit Applications Form can be found on the linked Alberta Soccer website.
Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to lfisher@calgaryminorsoccer.com.
Rosters
Any player listed on a Travel Roster that is NOT on the traveling team season’s roster, are required to have a Guest Player Form
Examples:
o Player is on with a different team, and within the same Club
o Player is with a different Club
Payment
When approval is confirmed, payment will be accepted.
Payment can be made in person by cash, cheque, Visa, MasterCard, debit card or over the phone during regular office hours by Visa or MasterCard.
